Order in Council 1601/2016

On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

PURSUANT T o subsections 7(1) and 7(2) of the Local Health System Integration Act, 2006,

Glenn Forrest of London

be appointed as a part-time member of the board of directors of the South West Local Health Integration Network to serve at the pleasure of the Lieutenant Governor in Council for a term of up to three years, effective the date this Order in Council is made.
