On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Order in Council numbered O.C. 1109/2016 dated June 30, 2016 and O.C. 583/2017 dated March 22, 2017 that appointed and designated

Janine Van den Heuvel of Troy

as a part-time member and part-time Chair of the Hamilton Niagara Haldimand Brant Local Health Integration Network under the Local Health System Integration Act, 2006 , be revoked effective as of the date this Order in Council is made.
