On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

as a Lawyer member of the Consent and Capacity Board under the Health Care Consent Act, 1996 , be revoked effective June 30, 2017.
