a) the name of the person required to pay the ad- ministrative penalty;

b) the provision of this Act or the regulations that the person contravened, a copy of the inspection re- port or order and the date on which the contravention occurred;

c) a description of the contravention;

d) the amount of the administrative penalty and the consequences for failing to respond to the notice;

Section 37 of the Act is amended 2 L’article 37 de la Loi est modifié (

a) by adding after subsection (1) the following:

Section 21 of the Workplace Health, Safety and Compensation Commission and Workers’ Compensa- tion Appeals Tribunal Act,

chapter W-14 of the Acts of New Brunswick, 1994, is amended 3 L’article 21 de la
