On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Pursuant to

section 9 of the Ministry of Health and Long-Term Care Act ,

Kelvin Chan of Richmond Hill

be reappointed as a part-time Professional member of the Committee to Evaluate Drugs for a period of three years, effective February 9, 2017 to and including February 8, 2020.
