On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:
Pursuant to subsections 7(1), 7(2) and 7(6) of the Local Health System Integration Act, 2006 , the persons specified in Column A of
Schedule 1 to this Order, be reappointed on a part-time basis to the positions as specified in Column B of
Schedule 1 to the board of directors of the Champlain Local Health Integration Network to serve at the pleasure of the Lieutenant Governor in Council effective April 1, 2021 to and including June 30, 2021.
