Section of the Workers’ Compensation Act, chap- ter W-13 of the Revised Statutes, 1973, is repealed and the following is substituted: 1 L’article de la

Section of the Act is amended L’article de la Loi est modifié (

a) by repealing subsection (4) and substituting the following:

a) the occurrence of an accident and the nature of it;

b) the day and the time the accident occurred;

b) des jour et heure de l’accident; (

c) the name and address of the worker who suffered an injury;

c) des nom et adresse du travailleur qui a subi une lésion; (

d) the place where the accident occurred;

d) de l’endroit où l’accident est survenu; (

e) the name and address of the worker’s attending physician or surgeon, if any; and

e) le cas échéant, des nom et adresse du médecin ou du chirurgien traitant; (

f) any other particulars prescribed by regulation.

a) a worker suffers an injury as a result of an accident that may entitle the worker or his or her dependents to

Part including loss of earnings and medical aid expenses but excluding first aid provided by the employer, or (

c) the employer receives a notice from a worker in accordance with subsection (6), if the employer has knowledge of the accident only by such notice.

d) in subsection (7) by striking out “notice shall” and substituting “notice to the employer by the worker shall”;

d) au paragraphe (7), par la suppression de « L’avis doit » et son remplacement par « L’avis que le travail- leur donne à l’employeur doit »; (

e) by repealing subsection (8).
