On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Order in Council numbered O.C. 1674/2015 dated December 2, 2015 that appointed

Cezary Paluch of Mississauga

as a Lawyer member of the Consent and Capacity Board under the Health Care Consent Act, 1996 , be revoked effective December 15, 2016.
