On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Pursuant to subsection 3(1) of the Ministry of Health and Long-Term Care Appeal and Review Boards Act, 1998,

be reappointed as a part-time member of the Health Professions Appeal and Review Board for a period of five years, effective November 14, 2017 to and including November 13, 2022.
