On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Order in Council numbered O.C. 1640/2016 dated November 2, 2016 that reappointed

Wendy Forsythe of Toronto

as a part-time member of the Physician Payment Review Board under the Health Insurance Act , be revoked effective the date this Order in Council is made.
