On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Order in Council numbered O.C. 864/2015 dated June 23, 2015 that reappointed

John Cochrane of Pointe au Baril

as a public member of the Board of Management for the District of Parry Sound West pursuant to the Long-Term Care Homes Act, 2007, be revoked effective September 28, 2016 as the member has submitted his resignation.
