On the recommendation of the undersigned, the Lieutenant Governor of Ontario, by and with the advice and concurrence of the Executive Council of Ontario, orders that:

Order in Council numbered O.C. 438/2015 dated April 15, 2015 that appointed

Jennifer Hamilton of Kitchener

as a member of the Consent and Capacity Board under the Health Care Consent Act, 1996 , be revoked effective as of the date this Order in Council is made.
