LOI SUR LA PHARMACIE PHARMACY REGULATIONS RÈGLEMENT SUR LA PHARMACIE The Commissioner, on the recommendation of the Minister, under

section 46 of the Pharmacy Act and every enabling power, makes the Pharmacy Regulations. Le commissaire, sur la recommandation du ministre, en vertu de l’article 46 de la

Loi sur la pharmacie et de tout pouvoir habilitant, prend le Règlement sur la pharmacie. Registration and Renewal Inscription et renouvellement 1. On application to be registered as a pharmacist under subsection 5(1) of the Act, an applicant shall, in addition to providing the information required by the Act, provide the Registrar with (

a) a mailing address and telephone number for the applicant; (

b) a photograph of the applicant clearly showing the applicant’s face, taken not more than six months before the application is made; (

c) proof that the applicant is entitled to work in Canada; (

d) information on the languages written and spoken by the applicant; (

e) information about the applicant’s prospective employment as a pharmacist in the Northwest Territories, including the planned date of commencement and the name, address and telephone number of the prospective employer; (

f) a curriculum vitae for the applicant including all employment history relevant to the practice of pharmacy; (

g) a copy of the applicant’s pharmacy degree and information about the educational institution that conferred the degree; (

h) information about (

i) each current and previous registration of the applicant as a pharmacist in other jurisdictions, (ii) any previous application for registration by the applicant as a pharmacist in the Northwest Territories, (iii) any refusal to grant the applicant a licence or permit to practice as a pharmacist, or refusal to register the applicant as a pharmacist, in the Northwest Territories or another jurisdiction, and (iv) any suspension or revocation of or restriction on the applicant’s right to 1. titre de pharmacien aux Territoires du Nord- Ouest, notamment la date d’entrée en fonction prévue, le nom, l’adresse et le numéro de téléphone de l’employeur éventuel;

f) son curriculum vitae, qui indique toute expérience professionnelle antérieure pertinente à l’exercice de la profession de pharmacien;

g) une copie de son diplôme de pharmacien et des renseignements relatifs à l’établissement d’enseignement qui lui a décerné ce diplôme;

h) des renseignements relatifs à, selon le cas : (

i) toute demande d’inscription actuelle et antérieure à

titre de pharmacien dans toute autre juridiction, (ii) toute demande d’inscription antérieure à

titre de pharmacien qu’elle a faite aux Territoires du Nord-Ouest, (iii) tout refus de lui accorder une licence ou un permis d’exercice à

titre de pharmacien, ou refus d’inscription à

titre de pharmacien aux Territoires 1

practice as a pharmacist in another jurisdiction; and (

i) information about any treatment received by the applicant for an addiction that may affect the applicant’s ability to practice as a pharmacist. On application for a renewal of a pharmacist’s licence under subsection 7(2) of the Act, the applicant shall, in addition to providing the information required by the Act, provide the Registrar with (

a) evidence satisfactory to the Registrar of the applicant’s identity; (

b) a mailing address and telephone number for the applicant; and (

c) information about the applicant’s employment as a pharmacist in the Northwest Territories, including the name, address and telephone number of the employer. 2. c) information about the applicant’s prospective employment as a pharmacist in the Northwest Territories, including the planned date of commencement and the name, address and telephone number of the prospective employer. 3. (1) The fee prescribed to register an applicant in the Pharmacists Register and issue a licence to the applicant under paragraph 5(4)(

a) of the Act is $317. 4.

(1) For the purposes of the Act and these regulations,

Schedule II is varied by removing the following substances from that drug schedule: (

a) naloxone hydrochloride injection, when indicated for emergency use for opioid overdose; (

b) naloxone hydrochloride nasal spray, when indicated for emergency use for opioid overdose. 6.1.

(2) A person who sells a substance referred to in subsection (1) by retail, or who supplies or distributes such a substance, shall ensure, when selling, supplying or distributing the substance, that adequate information on the appropriate use of the substance is available to the recipient.
