MELROSE DR., SUITE 130, VISTA, CA FOR COURT USE ONLY IN THE MATTER OF A NONMINOR DEPENDENT COURT APPOINTED SPECIAL ADVOCATE (CASA) ORDER OF APPOINTMENT OF CASA FOR NONMINOR DEPENDENT CASE NUMBER Having considered the facts presented and good cause appearing, the court orders that the above-named nonminor dependent be referred to Voices for Children, Inc. for appointment of a Court Appointed Special Advocate (CASA). IT IS FURTHER ORDERED THAT: 1. (name) is appointed as a CASA for the above-named nonminor dependent. The nonminor has agreed to the appointment. 2.

Said CASA shall have access to the records and files pertaining to the nonminor dependent which are maintained by the court, the Health and Human Services Agency (HHSA), the Probation Department, and any school in which the nonminor dependent is enrolled, and a copy of this order will be the only authorization necessary for such access. 3. The nonminor dependent may revoke their consent to said CASA’s access to confidential information at any time. 4.

Said CASA will not disclose the confidential information received to anyone other than the assigned probation officer, social worker, attorney(s), or the court unless specifically authorized by the court . 5. Said CASA will be given notice of and may attend all conferences and/or court hearings regarding the nonminor dependent. 6. Said CASA will receive all reports from HHSA and/or the Probation Department at least 48 hours prior to court hearings. 7.

Said CASA will, in coordination with the other parties, investigate and report to the court such information as will assist the nonminor dependent in achieving independence. 8. All of the work described above will be under the general supervision of Voices for Children, Inc. 9. This appointment will be in full force and effect until further order of the court or until Voices for Children receives a written request for the CASA’s removal by the nonminor dependent.
