s) you own or operate. The DMV, not the Court, will monitor the installation and compliance. The DMV will send you the following: • IID requirements, including the length of time it is to be installed and a list of IID manufacturers. • A Notice of Exemption form that is to be completed and submitted to the DMV if you do not own a vehicle. • Verification of Installation and Verification of Calibration forms that are to be completed and submitted to the DMV. Failing to comply may result in additional misdemeanor violations, a warrant, additional jail time and/or fines.

All questions regarding the Ignition Interlock Device should be directed to the DMV Mandatory Actions Unit at 1-916-657-6525. IMPORTANT Submit verification of installation, calibration, or exemption notices to the California Department of Motor Vehicles (not to the Court).
