section 340: IDENTIFYING INFORMATION: Name: Date of Birth: Social Security Number: AKA: Last known address: PHYSICAL DESCRIPTION: Gender: Race: Height: Weight: Hair Color: Eye Color: Tattoos/Scars: Other Information (optional): The Court, having read and considered the Declaration presented by the Social Services Agency (SSA), orders you to take the above-named minor into protective custody at any time of the day or night, and bring the minor before the Court. If the Court is not in session, deliver the minor to the custody of Orangewood Children and Family Center (401 The City Drive South, Orange, CA 92868).
