Justice Guy Lecompte on November 20, 2014; [ 3 ] Considering the testimony of the Petitioner and of the Mise-en-Cause; [ 4 ] Considering the evidence submitted by the Petitioner of the accomplishment of the conditions and formalities provided for under law; and [ 5 ] Considering that this Motion for Adoption is in the interest of the adolescent;

[ 6 ] THE COURT HEREBY: [ 7 ] GRANTS the Motion for Adoption ; [ 8 ] PRONOUNCES the adoption of the adolescent, X , by the Petitioner, A; [ 9 ] TAKES JUDICIAL NOTICE that the relation of filiation between the adolescent and the Mise-en-Cause, B, is maintained; [ 10 ] GRANTS the said adolescent the names and surname X ; [ 11 ] ORDERS the clerk of the Court of Québec, Youth Division, to give notice of the judgment of adoption to the Registrar of Civil Status of Quebec and to provide him with all the information required to draft a new birth certificate in English in accordance with

Article 129 of the Civil Code of Québec ; [ 12 ] ORDERS the Registrar of Civil Status to comply with the provisions established by law; [ 13 ] AUTHORIZES the clerk of the Court of Québec, Youth Division, to remit through the notary of the Petitioner, the originals of the documents he has filed in support of his Petition , namely his birth certificate, his marriage certificate and the adolescent’s birth certificate, as soon as the judgment on this Motion has been rendered and forwarded to the Registrar of Civil Status; [ 14 ] ORDERS the clerk of the Court of Québec, Youth Division, to retain a true copy of these documents in the Court file; [ 15 ] ORDERS the clerk of the Court of Québec, Youth Division, to issue and forward to the Petitioner a certificate attesting this judgment of adoption.
