[ 3 ] Considering the order of placement for adoption rendered on May 29, 2018 by Justice Anne-Marie Otis; [ 4 ] Considering the allegations of the application for adoption and the affidavit included; [ 5 ] Considering all the exhibits; [ 6 ] Considering that the child has adapted well to the applicant; [ 7 ] Considering the testimony rendered by the applicant; [ 8 ] Considering that the applicant has accomplished all the conditions and formalities prescribed by law; [ 9 ] Considering that the present application for adoption is in the best interest of the minor child; [ 10 ] Considering that the present application for adoption is well founded in facts and in law.

FOR THESE REASON, THE COURT: [ 11 ] GRANTS the Application; [ 12 ] PRONOUNCES the adoption of the minor child, X, by the applicant; [ 13 ] ATTRIBUTES to the child the following name and surname: X [ 14 ] ORDERS the clerk of the Court of Quebec, Youth Division to notify the present judgment of adoption to the Director of civil status and to supply all the information that will enable him to draw up a new birth certificate for the child in conformity with

section 129 of the civil code of Quebec;

[ 15 ] ORDERS the Director of civil status to comply with the provisions of the law; [ 16 ] AUTHORIZES the clerk of the Court of Quebec, Youth Division to withdraw and return to the applicants the original of the exhibits filed in support to the present motion, except for the original of the birth certificate of the child, exhibit D-2; [ 17 ] AUTHORIZES the clerk of this Court to give a copy of the present judgment to be rendered to the applicant.
