[ 1 ] GIVEN the application by A requesting the adoption of the teenager X, born on [...], 2009 in Town A, Quebec; [ 2 ] GIVEN the order of placement rendered on November 16, 2022; [ 3 ] GIVEN the evidence submitted by the applicant that the conditions and formalities established by law have been fulfilled; [ 4 ] GIVEN the testimony of the teenager who clearly indicates his consent to be adopted by the applicant; [ 5 ] GIVEN that the adoption is in the best interest of the teenager; [ 6 ] FOR ALL THESE REASONS, THE COURT:

[ 7 ] GRANTS the application for adoption; [ 8 ] PRONOUNCES the adoption of the teenager X, born on [...], 2009 in favor of the applicant, A; [ 9 ] MAINTAINS the filiation bond between the teenager and the impleaded party, B, mother of the teenager who also has parental authority over him; [ 10 ] ATTRIBUTES to the child the name and surnames of X ; [ 11 ] ORDERS the Clerk of the Youth Division of the Court of Québec to notify the Director of Civil Status of the adoption judgement, so that

an act of birth for the adopted teenager may be drawn up, in accordance with the prescriptions of section129 of the Civil Code of Québec ; [ 12 ] ORDERS the Director of Civil Status to comply with the prescriptions established by law; [ 13 ] AUTHORIZES the Clerk to give to the applicant the original of documents filed in the court records, with the exception of the birth certificate of the child but ORDERS him to keep certified true copies of the said document in the court records; [ 14 ] ALLOWS that a true copy of the judgment be forwarded to the applicant; [ 15 ] THE WHOLE without cost; Signed June 27 , 2023 __________________________________ LISA LEROUX, J.C.Q.
