[ 1 ] Considering the petition asking for the adoption of X born on [...], 2020 in Town A, province of Quebec; [ 2 ] Considering the placement order issued on October 2, 2021; [ 3 ] Considering the interest of the child and the proof given by the attorney of the petitioner of all the conditions and formalities established by the law; [ 4 ] ACCORDINGLY, THE COURT : [ 5 ] GRANTS the present application; [ 6 ] PRONOUNCES the adoption of the child X by A ; [ 7 ] ATTRIBUTES to the child the first name and name of X ; [ 8 ] ORDERS the clerk of the Quebec Court to notify the present judgment of adoption to the Director of Civil Status and to furnish him with all the information necessary to establish a new birth certificate according to the Quebec Civil Code ; [ 9 ] ORDERS the Director of Civil Status to conform to the prescriptions of the law; [ 10 ] ORDERS that the clerk of the Quebec Court sends a certified copy corresponding to the original of the judgment of adoption to the petitioner.
