[ 1 ] The Court is seized with a petition for adoption concerning X , born on […], 2010. [ 2 ] On March 19, 2014, an order of placement with the applicant-adopter, Mrs.

A, was rendered by Judge Lise Gagnon. [ 3 ] The child has been residing with the applicant-adopter since December 2011. [ 4 ] On March 19, 2014 a report from [Health and Social Services Center A]/Departement of Youth Protection A was filed as an exhibit on D-2.

This report shows that the child is well adapted to his life with the applicant-adopter. [ 5 ] The applicant-adopter has all the necessary qualities to exercise parental authority and to assume care, maintenance and education of the child. [ 6 ] The child to be adopted will be known as: X. [ 7 ] The present motion is in the child's best interest and respects his rights.

For these reasons the Court: [ 8 ] GRANTS the motion; [ 9 ] PRONOUNCES the adoption of the child by A; [ 10 ] ASSIGNS to the child to be adopted the following surname and name: X ; [ 11 ] ORDERS the Clerk of the Court to give notice of the judgment to the Registrar of civil status as soon as the judgment acquires the authority of a final judgment; [ 12 ] ORDERS the Clerk of the Court to give copy of the judgment to the attorney representing the petitioner as soon as the judgment acquires the authority of a final judgment; [ 13 ] AUTHORIZES the Applicant to withdraw the originals of the exhibits once judgment has been rendered and orders the clerk of the Court to remit them to the attorney representing the petitioner; [ 14 ] ORDERS the Registrar to make the required entries to ensure the publication of the registry.
